At Lake Regional, our goal is to provide exceptional patient care. As such, the following specific rights and responsibilities apply to patients throughout their hospitalization.

Human Dignity/Informed Decisions — Patients have the right to exercise cultural and spiritual beliefs that do not interfere with the well being of others. Consideration is given to care of the dying patient by optimizing comfort and dignity. We, as a hospital, have prime responsibility to assure that patient rights are preserved while under our care. To this end, the patient or the patient's legally authorized representative will be informed of their rights in advance of furnishing or discontinuing care.
Nondiscrimination — The patient shall be granted impartial access to treatment or accommodations which are available or medically-indicated regardless of age, race, creed, sex, national origin, religion or source of payment.
Refusal of Treatment — The patient, or appointed surrogate, has the right to refuse treatment, based on his/her own personal moral or religious beliefs, to the extent permitted by law, and to be informed of the medical consequences of his/her refusal.
Information — The patient — or in the case of a minor or incompetent adult, the patient's parent or guardian — has the right to receive from the practitioner responsible for coordinating his/her care complete and current information concerning his/her diagnosis, treatment and any known prognosis. This information should be communicated in terms the patient — or in the case of a minor or incompetent adult, the patient's parent or guardian — can reasonably be expected to understand.
Identity — The patient has the right to know the identity and professional status of individuals providing service to him/her, and to know which physician or other practitioner is primarily responsible for his/her care.
Privacy and Confidentiality — The patient has the right, within the law, to personal privacy and confidentiality, and to assume that all communication and records pertaining to his/her care be confidentially treated and read only by individuals directly involved in his/her treatment or the monitoring of its quality. The patient has the right to access his/her records within a reasonable timeframe.
Informed Consent — The patient — or in the case of a minor or incompetent adult, the patient's parent or guardian — has the right to a reasonable informed participation in decisions that involve his/her care; not to be subjected to any procedure without voluntary, competent and understanding consent, or the consent of a legally authorized representative or guardian; to be informed of, and voluntarily give or refuse consent to participation in, any human experimentation or other research/educational projects affecting his/her care or treatment; and to know who is responsible for authorizing and performing the procedures or treatment.
Safety — The patient has the right to expect a safe environment supported by the hospital's practices, facilities and equipment. This includes the right to access protective services, if needed.
Communication — The patient has the right to expect the hospital to be sensitive to their communication needs. This may include the use of large print materials, use of interpreter, or specialized programs for individuals who are blind or deaf.
Plan of Care — The patient, or designated representative, has the right to participate in the development and implementation of his/her plan of care. The patient has the right to expect reasonable continuity of care and assistance in locating alternative services when medically indicated.
Transfer — The patient has a right not to be transferred to another facility unless he/she — or in the case of a minor or incompetent adult, the patient's parent or guardian — has received a complete explanation of the need for the transfer, the alternatives to such a transfer, risks and benefits of the transfer, and acceptance by the other facility.
Services — The patient has the right to services appropriate for his/her age. These services will include furniture and equipment concurrent with a patient's size and developmental needs.
Hospital Charges — The patient — or in the case of a minor or incompetent adult, the patient's parent or guardian — has the right to examine and receive an itemized and detailed explanation of his/her bill, regardless of the source of payment. The patient — or in the case of a minor or incompetent adult, the patient's parent or guardian — has the right to timely notice prior to termination of the patient's eligibility for reimbursement by any third-party payer for the cost of his/her care.
Rules and Regulations — The patient — or in the case of a minor or incompetent adult, the patient's parent or guardian — has the right to know what hospital rules and regulations apply to his or her conduct as a patient.
Complaints/Grievances — The patient has the right to voice complaints regarding the care received without recrimination. Each patient will be informed of the grievance process. Patients will be provided with phone numbers of hospital, state and federal agencies, and The Joint Commission to report complaints/grievances.
Advance Directives — The patient has the right to formulate advance directives and have practitioners and staff provide care that is consistent with these directives.
Pain Management — Patient has the right to assessment and appropriate management of pain.
Notification of Family/Physician — If requested, the patient has the right to have a family member or representative of his/her own choice and his/her private physician notified as soon as can be reasonably expected after admission to the hospital. In such case that the requested party (parties) is unable to be contacted, an alternate individual(s) will be identified for notification. The patient has the right to designate specifically who should be the liaison for purposes of communication between family and/or significant others and the physician(s). The patient also has the right to specify who he/she does not want contacted. All such interactions should be documented in the patient's medical record.
Abuse/Harassment — The patient has the right to be free from all forms of abuse, exploitation or harassment.
Restraints/Seclusion — The patient has the right to be free from seclusion and restraints unless the restraint is used to limit mobility or temporarily immobilize a patient related to a medical, post-surgical or dental procedure.
Provision of Information
— Patients have the responsibility to provide, to the best of their knowledge, accurate and complete information about matters relating to their health. Patients also are responsible for making certain they clearly understand the medical treatment plan and what is expected of them.
Advance Directives, Religious Beliefs — Patients have the responsibility of informing their physician and the hospital of any advance directive (e.g. living will) or religious beliefs they wish to have honored in the event they become unable to speak on their own behalf.
Compliance with Instructions — Patients are responsible for following the treatment plan recommended by the practitioner primarily responsible for their care. Patients also are responsible for following the instructions of nurses and allied health personnel as they carry out their responsibilities and enforce the applicable hospital rules and regulations.
Refusal of Treatment — Patients are responsible for the consequences if they refuse recommended treatment or do not follow the practitioner's instructions.
Hospital Charges — Patients are responsible for ensuring the financial obligations of their care are fulfilled as promptly as possible.
Hospital Rules and Regulations — Patients are responsible for following rules and regulations affecting patient care and conduct.
Respect and Consideration — Patients are responsible for considerate behavior toward other patients, visitors and hospital personnel and for respect of their individual rights and property.
