Registration and Check-In

Patients scheduled for a procedure or test at Lake Regional may preregister online, in person at Lake Regional’s Patient Access Services office or by calling 573-348-8200.

All patients are required to check in at Patient Access Services upon arrival. At the point of check-in you will be asked to provide insurance cards and a valid driver’s license or photo ID for copying; sign consent forms; verify insurance information; provide your advance directive and/or living will for copying, if applicable; and make financial arrangements, if applicable. In addition, Medicare patients will be asked to complete the federally required Medicare Secondary Payor Questionnaire.

Information for Family Members and Loved Ones

Waiting areas are provided adjacent to patient treatments areas. Coffee, soda and snacks are available at the Expressions of Flavor Coffee Shops, located near the main lobby and the Emergency Department, during normal business hours. The cafeteria also is open to the public on the lower level of the hospital. Candy and other sundries can be purchased at the Wish-U-Well Gift Shops, located near the main lobby and the Emergency Department, during normal business hours.
