When a patient needs a copy of his or her medical records, Lake Regional’s Health Information Management (HIM) department is available to help. Patients may request either printed or electronic copies of their Lake Regional Health System medical records. Records may also be mailed to the patient or faxed to a physician.
NOTE: Records requests must be signed by the patient or the patient's legal representative and submitted in person, by mail or by fax. Records will not be released without signed authorization from the patient or the patient's legal representative. A valid photo idea is required to pick up records.
CHARGES: You may be charged a copying fee of $0.54 per printed page by HealthPort - the company contracted by Lake Regional to process requests. These charges are based on Missouri Revised Statute 191.227.
Requests will be processed within five to seven business days from receipt and may be picked up in person Lake Regional’s HIM office by the patient requesting the records or the patient’s legal representative. Records may also be mailed to the patient or faxed to a physician. If a delay is expected, you will be contacted at the phone number(s) and/or email address you provide.
Requests for Electronic Access to Medical Records
Medical records are available electronically through HealthPort — the company contracted by Lake Regional to process requests. To request online access to your records, print, fill out and sign the Electronic Record Delivery Request Form.
Upon receipt of your signed request, you will receive detailed instructions on how to access your electronic records via a secure web portal.
Requests for Printed Copies of Medical Records
To request printed copies of your medical records, fax or mail the correct Authorization for Use or Disclosure of Protected Health Information Form.
