This free, evidence based program has been developed by Stanford University to educate participants on self management of their chronic diseases, such as:

• Arthritis
• Diabetes
• Asthma and lung disease
• Chronic pain
• Heart disease
• Stroke
• Hepatitis
• High blood pressure
• Kidney disease
• Multiple sclerosis
• Parkinson's disease

The six week program will be led by professionals trained under the Stanford University model. Topics covered include problem-solving, goal setting, physical activity, nutrition, communicating with health professionals, managing emotions, medications and stress reduction. Participants will set goals and action plans to accomplish them each week, and will meet to discuss, share ideas and support one another. To view the flyer, click here.

During the first session, participants will recieve a free book and DVD and a free health screen which will include a complete blood count, complete metabolic panel, body mass index and A1C (if history of blood glucose problems). Refreshments will be provided each week.
